Q&A: Filling out disability and FMLA forms

Q: In a large medical practice, who typically fills out disability and Family Medical Leave Act forms for a patient? Is it best handled by human resources, the medical records department, registered nurses, or someone else?

A: It all depends on how hard the forms are to fill out. If all the information is easily accessible in the medical record, it can be done by a medical assistant, or even an experienced records person. It is never done by HR and usually not done by an RN. If the records are not complete enough to find the answers to the questions, it will take a very experienced clinician—most often the provider who saw the patient—to complete the forms.
