In 2003, the Canadian Forces (CF) drug benefit plan used preferential listing to control costs
associated with proton pump inhibitors (PPIs); however, significant expenditures for nonbenefit PPIs continued. A
database review was thus conducted to explain this anomalous usage pattern. This study provides further information
regarding the effectiveness of preferred listing as a cost-saving measure, noting that adherence to such policies
may be less than projected.

Gastroesophageal reflux disease (GERD) is known to cause sleep deprivation related to nighttime
heartburn or amnestic arousals during sleep. Recent findings suggest an inverse relationship of GERD symptoms and
lack of sleep, with sleep deprivation resulting in heightened perception of GERD symptom severity, according to a

In an aqueous setting, ascorbic acid in the stomach's gastric juice helps prevent the generation of

environment. However, when ascorbic acid is in the presence of a lipid in the stomach, it promotes nitrosation,
research published online Sept. 4 in Gut suggests.

The literature has well established that nonsteroidal anti-inflammatory drugs (NSAIDs) are very
effective in treating pain and inflammation, but these drugs are associated with significant gastrointestinal (GI)
toxicity. This is especially true in the elderly patient population. Selective cyclooxygenase-2 (COX-2) inhibitors
(coxibs) were developed to decrease the incidence of GI adverse events. Nevertheless, recent concerns regarding
coxibs and their association with adverse cardiovascular events have led physicians to re-examine the appropriate
use of all NSAIDs. Part 1 of this two-part article reviews essential data related to adverse GI events to help
physicians select appropriate patients to receive nonselective NSAIDs or coxibs. Other considerations, such as the
benefits of proton pump inhibitors (PPIs) and the mitigating effects of concurrent aspirin use, are also discussed.
Our clinical use pathway or algorithm will frame the discussion and guide clinicians through what has become a
difficult..

Cervical cytology, mammography, colonoscopy, and prostate specific antigen are several of the tools
available for cancer screening in healthy adults. Many individuals do not take advantage of these tests. In
addition, effective screening methods are not available for many types of cancer, such as ovarian cancer. It is
essential for physicians to listen to patients and note the type, onset, frequency, and severity of any symptoms,
which may indicate the need for a prompt workup and/or referral.

Approximately one-third of the adult population has hypertension. Large-scale clinical trials have
convincingly demonstrated that the treatment of isolated systolic and systolic/diastolic hypertension reduces rates
of total mortality, cardiovascular mortality, and stroke in older adults—yet control of systolic
hypertension remains poor. This article reviews systolic blood pressure as a cardiovascular risk factor and
discusses effective management of elevated systolic blood pressure. The role of combination therapy is emphasized,
focusing on the combination of a calcium-channel blocker and an angiotensin-converting enzyme inhibitor. In
addition to blood pressure-lowering efficacy, the combination of these two classes of metabolically neutral agents
may offer benefit in terms of tolerability and cognitive function.
